Schedule: PRN, require at least on average a shift a week with two shifts per month being a weekend day shift (Saturday or Sunday).
Shift: Days.Includes some weekends.
Location: This role will conduct patient home visits in the Lake, Sumter, and Marion County area.

The role you’ll contribute:
Provides occupational therapy services prescribed by a physician which can be safely provided in the home, assists the physician in evaluation of the patient's level of function by applying diagnostic and therapeutic procedures to guide the patient in the use of therapeutic, creative, and self-care activities for the purpose of improving function.
The value you’ll bring to the team:
·Performs an evaluation, assessing function using a method which objectively measures activities of daily living such as, but not limited to, eating, swallowing, bathing, dressing, toileting, walking, climbing stairs, using assistive devices and mental and cognitive factors, documenting the measurement results in the clinical record.
·Periodically reassess the patient every 30 days:provide the ordered therapy service, functionally reassess the patient and compare the resultant measurement to prior assessment measurements.Document the measurements in the clinical record along with the therapist’s determination of the effectiveness or therapy or lack thereof.
·Observes and records activities and findings in the clinical record and reports to the physician the patient’s reaction to treatment and any changes in patient’s condition, or when there are deviations from the plan of care.
·Instructs the patient and caregiver in care and use of assistive devices and the activity modifications; establishes a home program.

Certified Occupational Therapist Assistants, Home Health Aide
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
·Bachelor’s degree in Occupational Therapy
·Current state of Florida Occupational Therapy license
·Current Florida driver’s license
·Basic Life Support certification
Preferred qualifications:
·At least 2 years of clinical experience as an occupational therapist
·Experience working with adult and geriatric rehabilitation cases
·Home health experience within the past 3 years
·Current knowledge of homecare requirements and OASIS assessments
