The role you’ll contribute (Job Summary):

Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; linkage to resources in the community.

• This job description is not intended to be an exhaustive list of all duties. Employee may perform other related duties as assigned.
• Assess patient’s physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data.
• Evaluates patient’s response and intervenes to ensure optimal patient outcome.
• Develop and implement patient plan of care and observe outcomes in accordance with social
• work standards and in collaboration with the interdisciplinary team.
• Demonstrate competency in a variety of therapeutic/diagnostic interventions.
• Promote, advocate and collaborate to protect the health and safety and rights of each patient.

The expertise and experiences you’ll need to succeed:

Required: 

• Education: Master of Social Work (MSW)
• Cardiopulmonary Resuscitation (CPR)
• Current Basic Life Support (BLS) from American Heart Association for facilities with an Automated External Defibrillator (AED), preferred for facilities without an AED

Preferred:

• 6 months home care/hospice experience
• Two years post Master’s experience in health care or related field
