The community you’ll be caring for: AdventHealth Hospice Care
The role you’ll contribute:
The Admissions Nurse is responsible for responding to and visiting with patients who have been referred for admission to AdventHealth Hospice of Central Florida. The Admissions Nurse informs the patients, caregivers, and family members of the hospice services available with the organization and assesses patient for admission to services. In addition, the Admissions Nurse is responsible for visiting current Hospice patients who have been admitted to the hospital and communicates patient status with the respective interdisciplinary team.
The value you’ll bring to the team:
Meet prospect hospice patients at bedside. Different settings apply, such as private residences, community hospitals or Long-Term Facilities. Explain hospice benefits, communicate the program and services available through AHH of CFL to patients, care givers and families.
Perform physical assessment and review of records; collaborate with AHHC of CFL’s physician so to determine hospice eligibility. Obtain legal consent forms for hospice services. Assess immediate clinical / psychosocial needs. Determine patient’s level of care according to clinical acuity.
In a hospital setting, partner with hospital Case Manager to safely and efficiently plan the patient’s discharge; reviews medication reconciliation with AHHC of CFL’s physician, obtain orders and equipment and assess psychosocial needs. Develop the patient’s plan of care accordingly and initiate the admission of the patient into hospice.
In the LTC setting, partners with facility SW for the safe transition of the patient into AHHC of CFL care, review Medication reconciliation with MD, obtain orders and equipment and assess psychosocial needs. Initiate the admission of the patient into hospice.
In the private home setting, provide patient, family / care giver with thorough information regarding AHHC of CFL program, review medication reconciliation with AHHC of CFL physician, obtain orders and equipment and assess physical and psychosocial needs. Initiate the admission of the patient into hospice.
Accurately transcribe each patient’s admission and plan of care into AHHC of CFL’s electronic medical record, according to AH’s quality standards and guidelines. Communicate admission of patient into AHH of CFL services with patient’s primary care physician. Communicate admission of patient into AHHC of CFL to home team, to ensure a smooth transition of care.
Qualifications
The expertise and experiences you’ll need to succeed:
Education: Associates Degree is required; a Bachelors of Science in Nursing is preferred. An equivalent combination of education and experience may be substituted at the discretion of the Administrator.
Experience: Minimum of 3 years registered nurse experience in home health and/or acute care nursing experience; including 1 year experience in the hospital is required. Hospice experience is preferred.
Certificates and Licenses Required: A clear/active Florida Registered Nurse (RN) license is required, Certified Hospice and Palliative Care Nurse certification is preferred.
