Description:
The Certified Diabetes Educator (CDE) is a nurse or dietitian who has a minimum of 1000 hours of professional practice experience in diabetes self-management training, and has passed the Certification Examination for Diabetes Educators.
The CDE will implement clinical and education programs, be responsible for quality improvement and research activates, and be an expert resource for both patients and providers.
The CDE may also be responsible for the ADA Recognized Program process.
Provides consultation to inpatients, clinical patients, community residents and providers, facilitates communication between patients and providers and makes referrals to appropriate healthcare providers and community outreach
Provides 1:1 consultation within the clinic setting as needed in to cover department demands
This position is an inpatient role that would support BayCare Hospital Wesley Chapel and St. Joseph Hospital North
