Overview
St. Luke's Health System in Meridian, ID is seeking a Registered Nurse Care Advisor to join our Home Health team.
Shift:
Monday - Friday from 8:30am - 5:00pm with rotating evening/weekend call
Responsibilities
- Works with the patient (non-direct care), physician and care team, to create and implement a care plan that addresses the identified needs.
- Will serve as the primary contact, advocate and resource for patients, families and physicians.
- Assesses patient's knowledge of their clinical condition(s) and provide education and self-management support based on the patient's unique learning style.
- Will measure, improve and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served.
- Performs ongoing education to patient and patient's family and/or care giver on diagnosis, prognosis, treatment and medications.
- Serves as advocate when problems and issues arise.
- Perform other duties and responsibilities as assigned.
Qualifications
Required:
- Education: Bachelor's of Nursing Degree
- Experience: 2 years' RN experience
- Licenses/Certifications: Current RN License in the State of Practice
Preferred:
- Home Health experience and triage experience helpful
