JOB SUMMARY: An RN2 with the Hospital at Home Program;focuses on providing hospital-based care in the home setting; collaborates in establishing the home health plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum. The Registered Nurse analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis. Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes. Employs strategies to promote health and a safe and healthy environment. The responsibilities listed are a general overview of the position and additional duties may be assigned.
Job Description:
In the RN2 position you will be cross trained in three areas...Field Nurse, Monitor Tech and Intake Coordinator. Part and full time shifts are available.
Hospital at Home Program:
The Hospital at Home Department is a new area at Vanderbilt that cares for acutely ill patient in their home. This type of care would normally be provided in the acute inpatient setting. Patients admitted to the program have been evaluated in the hospital setting and admitted to services, but deemed appropriate to be cared for in the home.
SIGN ON BONUS:
$15,000 sign on bonus for new, full time RNs into this program. (Current VUMC employees do not qualify for the bonus. Prior employess must be separated from Vanderbilt employment for at least 1 year to qualify. Sign On Bonus paid out after 60 days of employment)
Position Shift:
Full and Part Time available
Part Time does not qualify for the sign on bonus
12 hour shifts
Rotating weekend coverage

The Home Health Quality team is a small task oriented team which consists of a quality educator, a quality data analyst, and quality reviewer. They work closely together with clinical managers to provide meaningful data on home health patient population and clinician best practices.
