Summary
In collaboration with the Trauma Case Manager, a MSW Trauma Social Worker develops, organizes, and implements individual discharge plans with the patient/family and medical team, focusing on the patient�s social, environmental, and psychological needs from a clinical framework. Responsible for assisting the patient and family to negotiate the role changes precipitated by the illness and hospitalization in order to reintegrate the patient back into the family system and community. Responsible for bereavement and grief counseling, as well as to engage the patient and family in the decision-making process to formulate a discharge plan. Coordinates the rehab plan, facilitates the interdisciplinary team conferences for the continuum of care plan. Assists with policy development, problem solving, program development and staff training.
Qualifications:POSITION QUALIFICATIONS:
EDUCATION:
Master�s Degree in Social Work or Counseling from an accredited university.
EXPERIENCE:
Prefer five (5) years in medical social work position at acute care level.
REQUIRED CERTIFICATION/LICENSURE/REGISTRATION:
BLS
OTHER QUALIFICATIONS:
Strong knowledge of countywide resources.
