Position Summary:
The Palliative Care LPN functions as a scheduler, consultant and clinician in the scheduling and delivery of palliative care services to patients; assists with triaging and coordination of consultations; gathers and maintains accurate program metrics. In collaboration with the multidisciplinary healthcare team, the LPN is responsible and accountable for the provision of educational opportunities to promote an optimal level of knowledge and performance skills for nursing personnel to enhance the quality of patient care. This position participates with staff debriefing and with the confronting moral distress. The Palliative Care LPN also develops, institutes, and maintains patient/family education and support material/programs as appropriate.
Minimum Requirements:
Experience in hospice, palliative medicine preferred
Education/Licenses/Certifications:
Current LPN license in the State of Washington
