JOB SUMMARY: The Vanderbilt Home Care Services, Home Health Aide/Certified Nursing Assistant will provide direct personal and supportive care for home care patients on an intermittent or hourly basis, based on physician orders. You will provide patient care and follow tasks as assigned on the patient's individual plan of care, designed by the Registered Nurse. The Home Health Aide reports directly to the Clinical Staff Lead. This position works in conjunction with other members of nursing service, ancillary services, physicians, patients, and families. It collaborates with various members of the healthcare team to ensure quality of care across the continuum. Requires the ability to be self-directed and quality focused. Interpersonal communication skills are a necessary function of this position.
Your Role:
As a Home Health Aide/Certified Nursing Assistant, you will travel to 4-6 patient homes throughout your shift, providing the prescribed care for the patient. Needing coverage of the North areas such as North Nashville, Cheatham, Robertson and Sumner counties.
Sign On Bonus - $4000(for full time position; paid out after 60 days of employment. Prior employees must be separated for one year before qualifying)
Position Shift:
Full time - 32 hour week
8 hour shift
No weekends

The Home Health Quality team is a small task oriented team which consists of a quality educator, a quality data analyst, and quality reviewer. They work closely together with clinical managers to provide meaningful data on home health patient population and clinician best practices.

KEY RESPONSIBILITIES:
Reviews and performs tasks as define on the personal care plan of the individual patient.
Responsibilities vary based on the care plan but may include direct assistance with activities of daily living, bathing, toileting, obtaining vital signs, intake and outputs, weights, transferring, provides facial shaving, oral care, hair care and dressing.
The Home Health Aide reports any abnormal findings to the nurse and/or CSL.
The Home Health Aide communicates with the clinical team in a timely manner.
The Home Health Aide works under the supervision of the Registered Nurse or Qualifying therapist.
The Home Health Aide adheres to practices and policies outlined to ensure patient safety and follows infection control standards.
