JOB SUMMARY: Assists in developing and meeting key Pillar outcomes and system improvement goals including financial, satisfaction, and clinical as the nursing component of the care coordination model. This position Improves outcomes by reducing all cause hospital readmissions and coordinating episodes of care among patients in a defined population or disease process. Participates in identification of appropriate patients; encourage patient and family engagement in self-care management; promote warm handovers to the next level of care by providing timely, pertinent information in a standardized way; conduct patient and family education on key elements of the patients' personal care plan by using the teach-back methodology and follow up phone calls; and assists the patient in navigating the healthcare system.
This position is for an oncology nurse navigator in the Vanderbilt-Ingram Cancer Center. You will have a non-clinical navigator who partners with you and there is also social work in the clinic.
This is a hybrid role- after training you will come to clinic once a week- the rest is remote.
The nurse navigator provides individualized assistance to disease-specific patients and their families in overcoming barriers to care, facilitating timely access to care, and providing education and support. The cancer patient's trajectory through diagnosis, treatment, and survivorship will be timely, high quality, and coordinated in collaboration with the healthcare team.
Assist with coordination of multidisciplinary visits as needed.
Act as a direct liaison for patients to assist with communicating clinical questions and concerns.
Contact patients prior to and after initial consult visit and throughout cancer continuum (including ED and inpatient visits) to assess patient needs, review treatment plan, etc.
Reinforces education for patients and their caregivers on disease process, treatment, side effects, ancillary services, and survivorship.
Advocate for patient and caregivers.
Maintain a list of patients.
Attend tumor board.
Communicates consistently with providers ensuring seamless plan of care and follow-up care.

Conducts primary care patient interviews and physical examination. Often takes a lead role in complex patient care situations. Possesses expert knowledge, training and experience to mentor less experienced peers.
