Outpatient Nurse Case Manager - Spinal Cord Injury and Disorder (SCI/D) Center
The Nurse Case Manager (NCM) provided evidence-based care, functions as the coordinator of the plan of care for outpatients in the Spinal Cord Injury and Disorder SCI/D Center, and coordinates hand off to the Inpatient NCM and/or Home Health Care Coordinator. The NCM collaborates with the patient/family and the interdisciplinary health care team with the goal to reduce admissions, inpatients days, decrease length of stay, and reduce number of urgent care visits.
