The Hospice Program Director is responsible for the overall management of hospice operations inclusive of the planning, staffing, organizing, directing and evaluating delivery of quality care. In addition to care delivery responsibilities the Director is accountable for the financial results of the program and collaborates with the business development staff to achieve growth goals.
Hours: Monday – Friday (40 hours) plus Administration on call every other weekend.
Primary Responsibilities of the Hospice Program Director:
Develops, implements, and evaluates policies and procedures as they relate to the delivery of hospice services and regulatory/accreditation requirements. Ensures adherence to applicable standards and maintains ongoing knowledge of any subsequent changes in standards.
Establishes interdisciplinary team members through direct employment or contractual arrangement.
Obtains appropriate contractual arrangements in order to provide all levels of care, i.e. in-patient general, respite, etc.
Establishes annual budget in collaboration with Finance Department. Understands financial metrics and manages care delivery to achieve a positive financial end result.
Establishes audit mechanisms to ensure financial and operational activities are timely and accurate, i.e. HISS submission, CTI completion, etc.
Acts in capacity of Supervisor of Clinical Services as needed.
Responsible for on-call and weekend staffing structure, ensuring communication on off hours is optimal.
Utilizes data such as outcomes, utilization practices, patient satisfaction and staff productivity to evaluate patient care and initiate process improvement strategies as appropriate.
Provides hospice orientation and education to professional and non-professional staff to meet regulatory requirements.
Schedules, coordinates and leads the Interdisciplinary Team Meetings at least every 14 days.
Ensures Interdisciplinary Team documentation meets regulatory requirements.
Promotes hospice and palliative care principles with both internal and external customers.
Participates in public relations and community activities.
Prior management experience
BSN with at least three to five years of supervisory/administrative experience in homecare or hospice or an MSN with one year of supervisory/administrative experience in homecare or hospice.
Strong proven leadership, management and communication skills.
Excellent interpersonal skills.
Creative and highly motivated facilitate program advancement.
Registered nurse with active valid license in the State of Connecticut
