This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more The VA Black Hills Health Care System is recruiting for a full-time, permanent Palliative Care provider in the Extended Care and Rehab service line. The following functional duties of the Professional Midlevel must be approved by the Medical Center Director, Chief of Staff, Clinical Executive Board. All the items listed are recognized competencies within the scope of practice for a graduate of a Physician Assistant Education Program accredited by the Council on Allied Health Education of the American Medical Association. Additional duties may be assigned by the Primary Supervising Physician, and appropriate Chief of Service Duties may include but not limited to the following: - Incumbent will provide facility-wide services assisting the Veteran and caregivers in navigating terminal illness, and with the transition to hospice when appropriate. - Manages a caseload through consultation from other providers. - Incumbent will be involved in "Concurrent Care Hospice" (CCH). CCH is for veterans who are utilizing community hospice services and are also using disease-targeted treatments such as but not limited to chemotherapy or radiation for palliative purposes. - The incumbent will participate in interdisciplinary meetings across Integrated Communities of Care (ICCs) to promote the best use of HPC services. - Adheres to administrative and clinical functions of the program. - Provides in-depth Veteran and family education to allow for informed decision-making. - Assist and conducts Veteran and family conference to determine goals for patients. - Identified symptoms causing physical and/or emotional distress and established plans of care to address needs. - Occasionally participates in the daily management of hospice and palliative care for Veterans in the Community Living Centers. - Demonstrates leadership in resolving complex ethical concerns that influence palliative care delivery. - Mentors and precepts staff as needed. - Maintains personal professional growth, knowledge and skills based on current literature through self-study and attended seminars and conferences with a hospice and palliative care focus. - Serves as an education resource expert on hospice and palliative care needs. - Serves as a role model for standards of professional practice. - Serves as a consultant for symptom management, establishing goals of care, and communication on end-of-life issues. - Identifies opportunities to collaborate with community organizations to increase awareness of health care services for hospice and palliative care needs. - Promotes public relations and outreach within the community served by the healthcare system. - Analyzes performance metrics and assists with the development of plans to drive program improvements. - Assists the hospice and palliative team in maintaining statistics and data required to meet the hospice and palliative care and quality performance metrics established by the VISN/VACO. - Initiates or leads quality improvement teams to collect data, analyze results, and design practice changes to enhance and evaluate health care outcomes.
