Boise VA Medical Center (BVAMC) is an interdisciplinary teaching facility with service-lines serving medical, surgical, inpatients and outpatients, with an Emergency Department, and multiple community-based outpatient clinics (CBOC). There is a large outpatient psychiatric population, with admissions for medical co-morbidities. Home-Based Primary Care is provided in the veteran's home. The RN works in Home Based Primary Care (HBPC) as a Case Manager. The RN is assigned to the Home Based Primary Care program (HBPC). The RN practices as a Case Manager (RN) within the VA Boise Medical Center. The RN functioning in the role of Case Manager is responsible for managing care across the health care continuum for a select group of clients. The RN provides professional assessment, coordination, telephone triage and planning of multiple health care services; acts on behalf of the veteran to assure that necessary clinical services are received and that progress is being made. The Case Manager RN is responsible to facilitate communication and collaboration between all members of the healthcare team involving the patient and family in the decision-making process; to minimize fragmentation of healthcare delivery; to optimize quality of patient and family outcomes in a cost-effective manner. The RN works effectively with all levels of staff, to include collaborative efforts with other health professionals providing ongoing case management services. Duties include (but are not limited to) Initial and continuous assessment for a culturally diverse veteran patient population in the home setting. Provides teaching to the patient and caregiver in ways of managing and delivering nursing care in the home. Monitors patient's condition and the care required in the home. Collects blood and other specimens required for monitoring the effectiveness of interventions and/or disease progression. Provides needed assistance and consultation for home care referrals. Assesses the care appropriate to the age related needs of patients served in the home setting.
