The Wilkes-Barre VA Medical Center is currently recruiting for a Registered Nurse for the Community Living Center. The CLC has oversight for the admission/approval process for nursing home patients and provide Nursing Care to Long Term care residents, dementia unit residents, respite patients, hospice patients, rehabilitation patients, transitional care patients and their families/significant others. The primary focus of the Community Living Center (CLC) is to provide a welcoming and functional home for the Veteran population. Rehabilitative, restorative, and supportive nursing care is provided to prevent deterioration whenever possible, to achieve and maintain optimal levels of functioning, independence and comfort, maximizing the Veterans ability to adapt and adjust to living as independently and safely as possible.The incumbent will be accountable for the continuum of care, patient, and family education, patient self-management after discharge and all supporting factors impact patient satisfaction. She/he will provide quality care to Community Living Center (CLC) residents in accordance with all VA standards and policies with special emphasis on physical, emotional, social, spiritual, cultural, and age-specific needs while preserving the patient's dignity and autonomy. Duties and knowledge required for this position include but are not limited to the following: Administer and document medication utilizing the computerized Bar Code Medication Administration (BCMA) program. Demonstrate sound clinical judgment and leadership in care improvement using the established nursing process and VA protocols. Demonstrate expert knowledge as it relates to respective nursing area and the ability to apply current theory to practice. Demonstrate excellent communication and critical thinking skills. Identify nursing diagnosis, clinical problems, and related interventions according to Nursing Services and/or unit policies. Utilize team-based approaches to identify, analyze and resolve issues; and maintain all unit specific competencies. Ensure the implementation of interventions to address identified patient care issues. Promptly report any changes in the patient's condition to the Medical Provider. Use creative approaches to proactively and safely distribute staff as needed by matching skill mix, skill level, patient acuity/census/workload, and available support services. Comply with all Medical Center protocols, policies, and standards of care.
