This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. The VA Southern Oregon Rehabilitation Center & Clinics (SORCC) provides a variety of clinical programs with a wide range of outpatient and residential treatment services for Veterans including primary care, psychiatry, substance use treatment, home based primary care, community care, choice, residential treatment, and various other ambulatory care programs. The VA SORCC includes a main campus and two Community Based Outpatient Clinics (CBOCs). The Social Worker will function in direct support of caregivers, Veterans and family members throughout the Facility and in the community and in collaboration with the Social Work Supervisor and Caregiver Support Team the Social Worker will support the development and implementation of General Caregiver services across the four counties served by VA SORCC. This includes development of a strategic plan for expansion of the General Caregiver Support Program and conducting frequent needs assessments to implement education, wellness, and support groups. The incumbent must be a skilled communicator capable of developing positive, effective relationships with Veterans, caregivers, local, state and federal agencies, as well as non-governmental organizations. The GS-12 Social Worker assists the Program Coordinator and/or the Program Manager in providing clinical services as part of the program, completing notes and gathering/monitoring data related to the day-to-day functioning and evaluation of the program. The Caregiver Support Social Worker has experience that demonstrates the advanced practice skill level in social work. At least two years of experience should be in an area of specialized social work practice. The Social Worker must be independently licensed by a state to practice at the advanced practice level. The Social Worker will adhere to the NASW Code of Ethics, all local social work policies, and maintain an unrestricted, unsuspended license to practice social work. The General Caregiver Support Social Worker's primary function is to provide direct clinical, evidence-based services and interventions to the caregivers and Veterans served by the Program. These clinical duties include, but are not limited to: Supportive individual counseling to Caregivers; Caregiver/Veteran couple's counseling when clinically indicated Caregiver support groups; these may occur at the facility and/or in the community Coordination of Care and brief Case Management to assist Veterans in accessing services in a timely manner; this may include direct referrals to VA medical and mental health treatment services and to other VA and Community services (ie, Home-Based Primary Care, Respite Program, etc..) including VA benefits, as well as to community-based services (ie., Hospice) and other non-VA entitlement programs. Clinical triage and crisis intervention when clinically required; this includes completing all aspects of VA required risk assessments and safety planning. Caregiver education workshops and training to Caregivers, VA staff and community partners regarding Caregiver issues. Community Outreach at both community (Veteran Service Organizations) and VA-sponsored events (ie., Mental Health Summit) regarding VA caregiver benefits, programs and services. Quality-improvement activities and evaluation/consultation with Social Work Supervisor and Caregiver Support Team.
