This social work position will serve as the primary contact, liaison and representative locally and at the state level to lead development of affordable housing options in the Alexandria VAHCS catchment area for homeless veterans. Will provide accurate and timely transactions and associations with the HUD-VASH program. The Social Worker makes independent professional decisions and recommendations with infrequent recourse to supervisory guidance. Will work collaboratively with HUD-VASH team. Overall responsibility for development of new and maintaining existing housing resources and grants for veterans to meet needs for safe and affordable housing. Primary responsibility for the development of assessments and gaps in special housing needs for disability populations and for proposing innovative solutions. Maximize existing housing resources and expand housing opportunities across disabilities in collaboration with community housing partners, stakeholders and advocacy groups. Negotiate with property owners and managers and play a lead role in developing housing options for the disable population. Maintains a housing directory, which should at a minimum, include documentation of all contact with housing providers, including name of contact, address, phone, date of contact and results of the contact. Develop and maintain current listing of available housing in the Oklahoma City VA Medical Center catchment area. Provide assistance to veterans in their housing search including: identifying available housing, contacting landlords to arrange tour of the property, provide transportation to veterans to tour property and assist the veteran in completing the lease application process. Assist the veterans in obtaining deposit assistance for HUD-VASH housing including: collection of required documents, matching the veteran with the appropriate funding source, follow-up with funding source to expedite payment to the landlord. Will oversee contacts with realtors, brokers/developers and landlords; preferential rents, renewals and terminations; maintain documentation and databases re: leases, rents, vacancies and tenant related activity; support and monitor HUDNASH Housing Choice Vouchers application process; oversee preparation of apartments for new tenants, inspections. Oversee apartment repair/improvements with landlords; assist with obtaining furnishing for veteran's apartments from community resources. Respond to veterans housing complaints and landlord complaints; coordinate with HUDVASH Case Managers and landlords; liaison with the local PHA to streamline the housing application and inspection process; monitor rent arrears and/or credits, including tenant notifications and oversee tenant housing/property legal actions in regards to legal evictions, as directed. Supports the overall mission of VA Homeless Program to eliminate homelessness among veterans. Documents patient information and all Individual Service plans in CPRS in accordance with the requirements of Medical Center policy and requirements of various accrediting bodies such as Joint Commission and CARF. The Social Worker will assist with GARF accreditation process and maintaining compliance. Employs casework methods and techniques to maintain a therapeutic relationship that will help the client work towards the best way of dealing with his/her problems. Participates in Social Work Intern and Student training. Such responsibility requires the ability to provide guidance and education to trainees about the Homeless Veterans Program. knowledge of mental illness, disabilities and best practice housing solutions for the disable population and supportive housing models. Knowledge of HUD rules and regulation related to housing, e.g. Oklahoma Landlord! Tenant law, Fair Housing Law, and reasonable accommodations. Knowledge of end of life issues including advance directives, palliative care, and grief. Serves on hospital committees and workgroups as assigned. Performs other related duties as assigned.
