This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more The Social Work Coordinator (FLOAT) Position is an advanced practice social work coordinator who will work in all Geriatrics and Extended Care Services and will float between the 120-bed Community Living Center and VANCHCS GEC Community Care programs, as the need arises. The VA Northern California Health Care System (NCHCS) GEC program Community Care Program administers and oversees community Care Services, including, Community Nursing Home (CNH), Adult Day Health Care, Respite, Homemaker/Home Health Aide, Home Based Primary Care, Medical Foster Home, Home Care Coordination, and other community care programs. Major duties include, but are not limited to: Develop a psychosocial assessment of the veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others. Develop the discharge plan, and setting achievable goals with the Veteran/family to conclude a discharge. Educate the Veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the Veteran's preference or that of his surrogate decision-maker. Evaluate consults for the Community Care Programs: CNH, ADHC, MFH, H/HHA , Respite, Home Care Coordination and other community care programs. Assess the appropriate services for the veteran based on his/her needs, the recommendation(s) of his/her VA providers, and the availability of contracted services. Functions as a Contract Officer Representative (COR). Working closely with Contracting Office to pursue contracts, monitor contracts, be a point person for referrals and admissions, and provide oversight of Veterans that are placed in these programs. The COR facilitates contract maintenance and/or new contracts in the community. As a COR, the social work coordinator works closely with the Office of Community Care and other Fiscal staff to ensure that bills are paid according to established procedure; this includes reviewing bills, releasing batches, approving 1358 requests, and problem- solving when needed to ensure smooth operation of this aspect of the program. The incumbent is aware of the budgets for each program, and makes program decisions accordingly, consulting with the SW supervisor and other staff as needed if changes to the program are necessary (for example, increasing or decreasing services).Provide education related to all programs in GEC service, as CLC/CREC and Community Care services to the appropriate VA providers for identified health education needs. Provide orientation and on-going training to social workers and interdisciplinary team members. Engage in regular program evaluation by developing and implementing QI monitors which examine an aspect of service which is relevant to the purpose of the program(s) and will lead to maintaining or improving services for the veterans served by the program. Develop a care plan along with the referring team, including short-term goals, long-term goals, veteran strengths, and veteran preferences. Collaborate with other service providers in reassessing the veteran's needs. Provide the veterans and their caregivers with ongoing supportive counseling.
