This Clinical Pharmacist serves in an acute care, medical and surgical hospital which includes a Nursing Home Care Unit, several Community Based Outreach Clinics, and the Jacobetti State Veterans Home. The hospital has a very active outpatient program and a highly specialized inpatient program. The incumbent is required to work during all shifts established for the position including weekends, holidays, and off tours. Major Functions: Clinical: Gives drug information to physicians upon request, and special instructions to nursing staff or patient on the storage, stability, and use of medications. Provides authoritative professional guidance on pharmaceutical problems of difficult, complex and unusual nature in addition to routine dispensing duties. Provides information to physicians and other staff members on availability of new drugs, warnings on currently marketed ones, contra-indications and incompatibility of other pharmaceutical compounds. Checks and/or prepares all intravenous solutions and admixtures using standards defined in the United States Pharmacopeia (USP). Checks and/or prepares unit dose syringes for IV injection. Responsible for other solutions which must be prepared under aseptic technique such as irrigating solutions and solutions used in inhalation therapy. Performs chart reviews when required and makes his/her recommendations in the progress notes. Routinely identifies customer concerns and performs the required tasks to resolve these problems in a timely manner. Dispensing: Review the patient's active and pending medication profile. Council to verbal education, patients are provided written educational information from the national drug database system as well as required medication guides for designated drugs. Comply with all legal requirements concerning counseling such as indications for use, directions and proper techniques, precautions and side effects, proper storage, drug interactions, what to do about missing doses, verifying allergy information. Responsible for checking the work of the technicians assigned to work in their area and assuring workflow is optimized to accomplish tasks. Compounds and dispenses medication by means of standard procedures in order to fill the written medication request of a physician, dentist, or other qualified prescriber. Performs those duties associated with the Unit Dose Medication Dispensing System. Administrative: Assists in special assignments, reports and projects, problem solving on matters that require an overall knowledge of pharmacy, and matters affecting the operation of the pharmacy. Makes recommendations to the Chief, Pharmacy Service on matters concerning budget and procurement and established policy. Reviews and records pharmacy stock needs and participates in the procurement of pharmacy supplies. Issues and accounts for controlled substances per medical center policy.
