Social Worker (Geriatrics & Extended Care Program Coordinator)
The Community Services Program Coordinator is an experienced social worker with proven clinical and administration skills who functions independently and has professional and administrative responsibilities for planning, organizing, and directing the Geriatric & Extended Care social work services within the medical center and those in the community. MAJOR DUTIES: The coordinator is responsible for day-to-day management and operational aspects of the H/HHA, ADHC, Veterans Directed Care, In-home Respite Services, Community Residential Care, and serves as the Co-Team Leader/Point of Contact for Charlotte Hall State Veterans Home.
