This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The Geriatrics Community Living Center Nurse Practitioner provides post-acute and long-term care to adults with have complex medical conditions who reside in our VA nursing home. As part of the Geriatrics Extended Care & Rehabilitation careline, the provider works as part of an interdisciplinary team and participates in care plan meetings. The Community Living Center Geriatrics NP also cares for Veterans receiving the hospice philosophy of care in our Community Living Center. The Community Living Center Geriatrics & Palliative Care Nurse Practitioner will: Provide direct patient/resident care to Veterans in the Community Living Center including admission H&P, all day-to-day aspects of patient care, discharge summary, participation in discharge planning and any family conferences. Provide direct patient/resident care to Veterans receiving the hospice philosophy of care. Occasional coverage for the other Geriatrics and Extended Care (GEC) programs may include: admission evaluations, daily care planning, chart reviews, Quality Improvement strategies, education etc. Participate in medical review of applications for admission to the Community Living Center. Other duties will be as assigned by the supervisor based on employee competence and organizational needs. Nurse Practitioner (NP) must have a minimum of a master's degree in nursing/nurse practitioner major, and board certification as an NP from a VA-approved certifying body required. As an advanced practice registered nurse (APRN), has prescriptive authority with a panel of patients. Core functions include diagnosing and treating acute and chronic health care problems, performing adult care check-ups, and prescribing medication and treatments.
