The Registered Nurse with Vanderbilt Home Care Services, applies the nursing process to plan and implement the care of patients, including patient and family education and continuum of care planning. As a Registered Nurse with the intermittent team, you will visit approximately 4 to 6 patients at their homes throughout your shift, providing the best patient care. The patients will be within 3-5 zip codes. Pediatric patients may be included. There is a mileage allowance with this position. The Registered Nurse, Home Care effectively communicates pertinent patient/family information to the health care team. Collaborates in establishing patient goals and evaluates progress to ensure effective outcomes. Furthers professional growth by engaging in learning activities according to the Board of Nursing standards.

The Home Health Quality team is a small task oriented team which consists of a quality educator, a quality data analyst, and quality reviewer. They work closely together with clinical managers to provide meaningful data on home health patient population and clinician best practices.
