As a member of the interdependent team led by a designated RN, the LPN is accountable to perform patient care duties and activities for a group of Hospice patients.
Consults with the Nurse Case Manager as needed to make modifications in the plan of care, and obtains physician orders for all changes made in the plan of care. (Physician orders must be discussed with the RN Case Manager).
Counsels the hospice patient, family and caregiver regarding care needs and health promotion/prevention and reinforce teaching/education at the direction of RN.
Monitors the patients progress and responses to care/services delivered, and update the Nurse Case Manager, physician and other health care team members as indicated.
Maintains complete, accurate and timely clinical records/documentation on all patients according to agency policy.
Stays current (expert) in home care and hospice regulation and all applicable licensing.
Actively participates in educational programs, meetings and agency committees as appropriate.
Accepts responsibility for the understanding and implementation of the discipline applicable content of all departmental communications and procedures.
Identifies processes to improve and potential solutions.
Licensed as an LPN and eligible to practice in Minnesota
Current CPR (BLS for Health Care Providers) certification
Six months recent experience in a med/surg, long term care, or home care/hospice setting as a CNA, HHA or LPN.
Two or more years recent LPN experience in a med/surg or home care/hospice setting.
Valid Minnesota drivers license (or license that matches your current permanent residence)
Current and adequate automobile insurance policy
Computer skills (able to document effectively in EMR)
