SUMMARY OF JOB RESPONSIBILITIES:
The Primary RN
Provides nursing care and oversight to patients in hospice care that are assigned to their caseload.
Develops and implements the plan of care for patients to honor their goals and wishes during their end of life process, aiming to make the experience meaningful and helpful to both the patient and family.
Demonstrates excellence in nursing care, using clinical assessment and disease state knowledge and experience to support the patient throughout the illness trajectory.
Provides direct care and support to the patient and family in collaboration with the Interdisciplinary Team (IDT).
Oversees the care of the whole patient, not just their hospice diagnosis.
RN with current Colorado licensure; graduate from an accredited school of nursing. Bachelor degree preferred.
Current CPR required.
Two to three years of acute care RN experience (ideally in a hospital), or comparable nursing experience.
The physician ability to work in a variety of settings in terms of stairs, temperature, and emotional climate.
Unlimited access to reliable transportation.
Adequate automobile insurance as required in Colorado.
Reads, writes and speaks English in a manner that can be understood by others
PHYSICAL REQUIREMENTS – Medium work
While performing the duties of this job, the employee is regularly required to stand, walk, sit, use the hands to finger, handle or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell.
Must be able to lift and/or move up to 50 pounds and push/pull up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

With only six employees, we cared for 81 patients and their families in 1993 – our first year of operation. Today, we serve more than 7,000 square miles with 5 offices and a state-of-the-art HopeWest Hospice Care Center. With a staff of 370 and more than 1,300 volunteers, care is provided to more than 2,500 patients and well-over 1,500 individuals coping with grief every year in Mesa, Delta, Montrose, Ouray and Rio Blanco Counties.
HopeWest is so much more than a hospice or health care organization. We are a hospice, palliative care organization, counseling program, and social program providing support to people in their homes, hospitals, and assisted living residences – regardless of age ...or illness.
The organization provides extensive bereavement services for people of all ages. Adults receive individual counseling, grief education and group support. HopeWest Kids is the only program of its kind in western Colorado providing an extensive array of creative grief services, such as: Itty bitty Camp for preschoolers, Camp Good Grief for older children and Teen Retreats for teens, equine therapy, and grief support groups in all grade schools, middle schools and high schools across the region.
We are woven into the communities we serve through our Board of Directors, Foundation Board and Advisory Boards in every community where we have an office. Philanthropy is key to helping support the mission and is raised through grants, individual and corporate gifts, events and support from our retail businesses – Spoons bistro & bakery, Heirlooms for Hospice and Artful Cup.
