The Clinical Risk Management RN is responsible to work collaboratively with others to support an effective transition for patients being discharged from the hospital who are at risk for readmission with the goal of achieving optimal patient outcomes and reducing hospital readmissions. The Registered Nurse is responsible for ongoing care management coordination through the continuum of care. This individual supports managing complex patients by supporting treatment plans and facilitating patient’s access to appropriate health care services, community resources, in addition to supporting patient self-management, symptom management, and increasing knowledge of his/her disease process. The Nurse supports the physician’s practice by following physician treatment plans to manage patient’s therapy, related education, triage and coordination of care and referrals when appropriate. In addition to working with the patients and their families, the Nurse interacts regularly with physicians and personnel from within the Marshfield Clinic Health Systems (MCHS) and outside to include, but not limited to: Pharmacies, clinic/office/hospital personnel, laboratories and home care nurses to coordinate care of patients.

Bachelor in Science of Nursing (BSN)
Two years’ recent clinical nursing experience in acute or ambulatory care; plus five years recent clinical experience in acute or ambulatory care preferred
Prefer experience with patient interaction on the telephone. Involvement with unit-based quality management.
Current and unrestricted Wisconsin Registered Nurse Licensure
Care Management RNs working in the Anticoagulation Service are required to obtain Anticoagulation Management Certification within six months of hire.
Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.
