Responsible for the day-to-day clinical operations overall coordination and administration of the hospice program and responsible for the compliance of clinical services with all state and federal hospice regulations. Exercise general oversight of the delivery of hospice care. Promotes cost-effective and clinically excellent hospice care. Manages professional growth and job satisfaction for the hospice interdisciplinary group (IDG) including as well as the management functions of planning, organizing, directing, and controlling. Maintains communication between hospice, hospital administration, and other hospital departments and care partners including nursing homes, assisted living facilities, group homes and physician practices. All activities will be performed in support of the strategy, vision, and values of Phoebe. Actively participates in the hospice’s Quality Assurance and Performance Improvement (QAPI) program.
Facilitates staff and Interdisciplinary Group (IDG) meetings and ensures the IDT members complete mandatory in-services and requirements and competency evaluations on time.
General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels.
May be exposed to limited hazardous substances or body fluids, or infectious organisms.
May be required to change from one task to another of different nature without loss of efficiency or composure.
Periods of high stress and fluctuating workloads may occur.
Long-distance or air travel as needed - not to exceed 10% travel.
4 year / Bachelor's Degree in Nursing or Health Administration (Required)
Master's Degree in Nursing or Health Administration (Preferred)
Have near normal hearing; Hear alarms/telephone/tape recorder/normal speaking voice
Have near normal vision: Clarity of vision (both near and far), ability to distinguish colors
Have good manual dexterity
Have good eye-hand-foot coordination
Occasionally within shift (1-33%) : Standing,Walking,Sitting,Climbing,Bending/Stooping,Twist at waist,Pushing/Pulling,Lift/carry up to 20 lbs,Lift/carry > 20 lbs with assistance,Reaching above shoulder.
Certifications and Licensures
Required Certifications/Licensures : Registered Nurse (RN) with current Georgia license,Certification in Hospice and Palliative Nursing within 3 years.
