NADAP, a recognized provider of wraparound services supporting the health and recovery of New Yorkers for 45 years, seeks applications from dedicated, passionate professionals for the Care Coordination Supervisor position in their fast-growing Health Homes Care Coordination Program.
The Care Coordinator Supervisor position is an exciting opportunity to manage a team and make a significant contribution to improving the health of vulnerable New Yorkers.
The Care Coordination Supervisor manages a team of Bachelor-level Care Coordinators who support individuals in achieving short and long term health care goals using a voluntary, person-centered model. The Care Coordination Supervisor works closely with his/her team to ensure consistent provision of high quality health care coordination to clients in the community who have chronic medical and/or mental health conditions. The Care Coordination Supervisor is a member of diverse supervisory team, and will work closely with other supervisors and managers to develop, train, and coach the direct care staff. The Care Coordination Supervisor provides clinical supervision to non-clinical staff via targeted trainings, one-on-one supervision, group supervision, and case review of critically ill or especially challenging clients.

Master’s Degree in Social Work, Mental Health Counseling, Public Administration, Public Health, or a related field.
One year of job-related experience supervising staff who deliver medical, mental health, case management, or addiction focused services to individuals or families.
Desire to work in a collaborative, team focused environment.
Desire to innovate, and instigate positive change in policy, staff, and client care provision.
Working knowledge of medical and behavioral health care systems strongly preferred.
