Incumbent is responsible for coordinating activities concerning "Outpatient Palliative Care." This individual will be the "go to" person for palliative care outside the hospital. "Outpatient Palliative Care" includes Home Palliative Care, Outpatient Palliative Care seen in the Family Medicine clinic and in other clinics (possibly including Geriatrics, and other palliative clinicians), palliative care provided by an clinic-embedded Palliative Care Nurse Practitioner (NP) collaborating with a Palliative Care physician to carry out advance care planning and palliative care, and other palliative care programs developed in the outpatient setting. This individual is also responsible for coordinating activities between Outpatient Palliative Care and the inpatient Palliative Care team and Discharge Planning at SM-UCLA and RR-UCLA medical centers, the advance care planning social worker in RR-UCLA medical center, and other programs of the Advance Care Planning and Services program. This individual also will be responsible for coordinating palliative care for high risk patient populations identified through risk stratification lists and other mechanisms. Incumbent will work directly with practice managers, physicians and nurse case managers in supporting patients and preventing unnecessary hospital admissions or ER visits.
Experience with palliative and hospice care resources and operations highly preferred. Knowledge of end of life care including advance care planning and POLSTs. Ability to perform Microsoft Word and Excel with accuracy and appropriate formatting. Skill in reading and understanding documents written in Standard English text for informational communication, action and distribution. Writing skills to compose, edit and proofread routine business correspondence using appropriate English grammar, syntax, spelling and punctuation. Oral communication skills to speak with individuals of varying backgrounds to obtain and convey specific information. Ability to establish and maintain cooperative working relationships with physicians, staff, patients, and administrative personnel. Ability to independently analyze information or problems, identify relevant factors, trends, and options, then implement and monitor solutions. Ability to perform basic mathematical calculations quickly and accurately. Skill in organizing and prioritizing workload in order to meet deadlines in light of changing, heavy workload and competing requirements. Ability to perform job duties in a detailed and accurate manner. Skill in performing with frequent interruptions and/or distractions. Working knowledge of the organization and where to go to get issues resolved. Working knowledge of University policies, procedures and forms, as they relate to performance of job duties. Knowledge of medical terminology. Knowledge of insurances and prior authorizations. Ability to speak conversational Spanish preferred.
