Office based position under direction of the Director Continuum of Care Business Operations. Responsible for analysis of SOC, Recertification and Resumption (and Other Follow-up OASIS) OASIS and accompanying documentation, management of proper utilization of resources to ensure positive patient outcomes and optimal financial outcomes. Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Home Health Team, and are expected in this role. Duties will include covering team member PTO, and other requested office tasks.
Education: Graduate of an accredited school of professional nursing or physical therapy
Experience: Qualified individuals will successfully complete the competency assessment; two year Home Health case manager experience
Training: Home Care Coding Certification required within 12 months of hire
License: Preferred Licensed or eligible for RN or PT licensure in the State of
Florida Other: Communication and interpersonal skills, basic computer skills, and customer service skills required
